Ecuador has relatively permissive pharmacy regulations — many medications that require a prescription in Europe or North America are sold over the counter (OTC) here. This is both useful (easier access) and a public health concern (potential for misuse). For visitors and new residents, understanding what's available and what to watch out for is important.

What You Can Usually Buy Without a Prescription

  • Antibiotics: Many types available OTC at farmacias, including amoxicillin, azithromycin, ciprofloxacin, and metronidazole. While officially controlled, enforcement is inconsistent. Note that self-prescribing antibiotics carries risks — wrong antibiotic, wrong dose, antibiotic resistance.
  • Anti-parasitic medications: Albendazole, metronidazole, tinidazole — useful for traveler's diarrhea and intestinal parasites
  • Antifungals: Topical and oral; fluconazole (Diflucan equivalent) typically available OTC
  • Antihistamines: Loratadine, cetirizine, diphenhydramine — all available
  • Painkillers: Paracetamol (acetaminophen), ibuprofen, naproxen — widely available
  • Oral rehydration salts (suero oral) — essential for any diarrheal illness
  • Anti-diarrheal: Loperamide (Imodium)
  • Blood pressure / diabetes medications: Available for established patients; may be asked for a prior prescription
  • Contraceptives: Available OTC at most farmacias

Controlled and Restricted Medications

Some medications are more restricted:

  • Opioids (tramadol, codeine, morphine) — require official receta retenida (retained prescription) from a registered doctor
  • Benzodiazepines (diazepam, lorazepam, alprazolam) — controlled; prescription required and may be retained
  • Stimulants / ADD medications: Limited availability and controlled

If you take controlled substances prescribed from your home country, bring enough supply plus a copy of your prescription. Importing personal-use quantities of prescription medications is generally allowed, but keep documentation.

Availability of Specific Medications

Not all medications available in your home country will be available in Ecuador, or may be available under different brand names. Generic (genérico) equivalents are usually available and are significantly cheaper.

If you have a specific chronic condition:

  • Research the generic name (not brand name) of your medications before you arrive
  • Confirm availability at a local farmacia before running out
  • Keep a reserve supply from home when first arriving

Bringing Medications from Abroad

Personal-use quantities of prescription medications are generally allowed through Ecuadorian customs without issue when accompanied by documentation. There is no strict quantity limit defined publicly, but carrying a 3–6 month supply of a necessary medication is typically accepted. More than 6 months' supply of controlled substances may attract scrutiny.

Best practice:

  • Carry medications in original packaging with pharmacy labels
  • Carry a copy of the prescription or doctor's letter (in Spanish if possible)
  • Keep medications in carry-on luggage, not checked bags
  • Declare at customs if you have significant quantities of controlled substances

Medication Costs

Generic medications in Ecuador are very affordable. Brand-name imported medications are considerably more expensive and may not be stocked at smaller farmacias. The IESS pharmacy provides medications free for affiliates through their formulary.
