One of the most common questions for expats and new residents on the Peninsula is: should I use IESS (public healthcare) or private clinics? The honest answer is: ideally both, but the right balance depends on your situation.

IESS (Instituto Ecuatoriano de Seguridad Social)

IESS is Ecuador's public social security system, which includes healthcare. Affiliates receive free (or very low cost) medical attention at IESS hospitals and clinics.

Pros:

  • Free or near-free for affiliates
  • Covers major surgeries, hospitalizations, medications
  • Dental services included
  • BIESS (IESS bank) offers low-rate mortgages to affiliates

Cons:

  • Long wait times (weeks or months for specialist appointments)
  • Quality of service varies significantly by facility and specialty
  • IESS facilities on the Peninsula are limited; complex cases go to Guayaquil
  • Not available to non-affiliated residents (foreigners on tourist visa cannot access IESS)

Private Healthcare

Private clinics and insurance offer faster access and more comfortable facilities.

Pros:

  • Much shorter wait times (same-day or next-day appointments common)
  • More modern facilities in many cases
