The Instituto Ecuatoriano de Seguridad Social (IESS) is Ecuador's public social security system. It provides health insurance, pensions, unemployment benefits, and other social protections to workers in formal employment and their families, as well as voluntary affiliates.

What IESS Covers

For affiliated members (afiliados), IESS provides:

  • Medical care — outpatient, specialist, emergency, surgery, maternity, dental (limited), mental health
  • Medications — through IESS pharmacies and dispensaries
  • Hospitalization — in IESS hospitals and through contracted facilities
  • Disability benefits — for work-related and non-work-related disability
  • Maternity leave payments and childcare
  • Retirement pension — after meeting contribution requirements
  • Unemployment insurance — for formal employees who lose work
  • MontepĂ­o — survivor benefits for dependants of deceased affiliates
  • QuirĂșrgico — surgical fund contributions

Who Can Affiliate

Mandatory affiliation

Any worker in formal employment in Ecuador must be registered with IESS by their employer. If you are employed under a legal contract, your employer deducts your contribution from your salary (approximately 9.45% of your monthly wage) and pays an additional employer contribution (approximately 12.15%).

Voluntary affiliation

Self-employed workers, freelancers, informal workers, and some foreign residents can affiliate voluntarily. Voluntary affiliates pay both the employee and employer portions themselves — roughly 21.6% of their declared income per month. This is a significant cost but provides full access to IESS health services and contributes toward pension rights.

Pensioners and retirees

Retired IESS affiliates continue to receive health coverage as pensioners (jubilados).

How to Access IESS Services

  1. You must be an afiliado activo (active affiliate) — check your status on the IESS website or app
  2. For non-emergencies: request an appointment (turno) through the IESS app or website, or in person at the dispensary
  3. For emergencies: go directly to the IESS emergency area; they are obligated to attend regardless of appointment status
  4. Bring your cédula de identidad (or passport for foreign affiliates) to every appointment
  5. Prescriptions are filled at the IESS pharmacy — bring your cĂ©dula and the digital prescription

For Employers

If you hire anyone formally in Ecuador, you are legally required to register them with IESS within 15 days of starting work. Failure to do so carries significant fines and penalties. Registration is done online through the IESS employer portal. See Working in Ecuador for details.
